About This Service

Hope Haven’s Crisis Stabilization Services are located in Burlington, Iowa. This program offers Crisis Stabilization to up to five individuals who are experiencing significant incapacitating or debilitating disturbances in mood or thought which interferes with activities of daily living to the extent that immediate intervention is necessary.

 

To be eligible for this service the individual must be an adult with a diagnosis of mental illness, intellectual disability or development disability and be living in the 8-county SEIL region.

Individuals may stay in the program for a maximum of six weeks.

  • Assistance in establishing linkages to referral programs, service providers, medical care and appropriate treatment programs

  • Personal support services and assistance in meeting basic human needs

  • Medication support and monitoring

  • Housing and living arrangements

  • Crisis intervention

  • Social rehabilitation

  • Peer support

  • Social skills, leisure time education, and vocational rehabilitation

  • Family and community support, assistance and education

  • Protection and advocacy

  • Service coordination

  • Transportation
